It's a needle, the scar will be really small and not easily picked up on camera. He didn't catch the bullet with his bare teeth, he had a metal cup sitting in his mouth that could withstand the shot and he also used a mouth guard which his throat actually got injured from during rehearsal (it was also not a very powerful round)
